Forms
To enable Heinz Chapel staff to make the final preparations for your wedding, you must complete the following forms and mail them to the Heinz Chapel office.
Wedding Application
The wedding application (PDF) must be received by the Heinz Chapel office no later than two months prior to your wedding date.
Music Selection Form
The music selection form (PDF) must be received by the Heinz Chapel office no later than one month prior to your wedding date.
The wedding application must be completed on hard copy, but the music selection form may be submitted online.
